Work Shift:

Responsibilities:
Responsible for walk‑in and telephone triage.
Coordinates patient care through collaboration with the patient, family, and healthcare team.
Provides patient care by assessing, planning, implementing, evaluating, and documenting individual patient needs, actions taken, and patient responses.
Responsible for case management based on PCP panels, in collaboration with network providers, including nurse visits and telephone management.
Delegates or assigns aspects of patient care to healthcare team members according to their validated competency.
Completes documentation within scope of practice.
Performs other functions as required.
Qualifications:
Licensure:
State of New Mexico or Compact State Nursing License Degree from accredited academic nursing program (If a program is under conditional status, approval may be granted by system CNO)
Minimum ADN preparation with expectation of BSN completion within 5 years of hireBLS required
