Job Overview
RN Case Manager with previous Home Health experience needed for a 13‑week contract role in Somerset, Kentucky. Pay: $50 per hour. Shift: Monday through Friday, with on‑call weeks (start Friday, end Thursday).
Location & TravelMileage reimbursement: 0.43 cents per mile. Counties covered:
Pulaski County (Somerset is the city).
License:
Kentucky. Must hold a current, unrestricted state Registered Nurse license. Ability to pass background check, drug screen, and have updated immunizations.
Recent Home Health experience preferred.
Primary ResponsibilitiesPerform comprehensive assessment of high‑risk members; collaborate with primary care providers to implement individualized, comprehensive care plans; assess and manage cost‑efficient, appropriate levels of care; provide non‑medical support such as housing and transportation; coordinate care transitions; engage physicians, social workers, discharge planners, and claims professionals; engage specialty resources; keep detailed records of clinical, functional, and fiscal outcomes; identify opportunities for health promotion and illness prevention;
prevent adverse patient occurrences and intervene promptly; act as patient advocate protecting privacy and confidentiality.
- Associates or Bachelors Degree in Nursing or related field.
- Ability to analyze complex medical information and make rational decisions.
