Licensed Practical Nurse (LPN) – Float Nurse, Family Practice (Full Time, Monday‑Friday, 8AM‑5PM)

• Assist in patient assessment by collecting data for analysis by an RN.
• Perform plan‑of‑care interventions, including specimen collection and wound care.
• May administer medication as required.
• Document patient care provided.
• Assist RN with development of care plans.
• Assist patients with activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.
• Monitor, document, and communicate patient's status and reactions to interventions with RN, physician, patient, and families.
• Act as an advocate for the physical and emotional well‑being of the patient.
• Schedule appointments and procedures as ordered by the provider.
• Obtain insurance re‑authorizations for testing and scheduled procedures.
• Attend clinic meetings.
• Maintain a clean and safe working environment.
• Report incidents, hazards, drug interactions and initiate appropriate action.
• Complete safety instructional programs.
• Assist other team members when time permits.

• Graduate of an approved practical nursing program.
• Current LPN/PN license to practice in the Commonwealth of Kentucky and in good standing with the Board of Health Professions.
• BCLS certification within 30 days of hire.
