Assesses, and plans interventions to help patients cope with social, emotional, economic, and environmental problems.
Responsibilities:- Interview and assess patients and/or patients’ family, caregivers, and/or legal representatives.
- Determine, prioritize, provide and/or arrange for needed internal and external services/interventions.
- Participate in case reviews to evaluate case management and progress.
- Consult with healthcare team members to promote, monitor, and evaluate compliance with patient’s treatment plan.
- Assist with discharge planning and processes, identify appropriate resources (transportation, housing, healthcare, and social/spiritual services), and provide referrals as part of the discharge plan.
- Supervise and/or train new staff, students, and interns.
- Provide individual, family, and group therapy.
- Bachelor’s degree required;
Master’s preferred. - Handle With Care Training within 30 days of hire.
- Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.
- Must be able to work in a stressful environment and take appropriate action.
