Position Summary
Case Manager (MLTSS/D‑SNP Programs). The Case Manager is responsible for developing and implementing healthcare strategies to ensure quality care for members, particularly those in Managed Long‑Term Services and Supports (MLTSS) and Dual Special Needs Programs (D‑SNP/FIDE).
Key Duties- Conduct in‑home assessments using comprehensive tools.
- Create and manage individualized care plans.
- Coordinate services such as adult/pediatric daycare, personal care, and nursing facility placements.
- Collaborate with members, families, providers, and interdisciplinary teams.
- Ensure cost‑effective service delivery and accurate documentation.
- Mentor new hires once they are proficient.
- Travel 50–75% within assigned counties.
Advanced clinical judgment, critical thinking, strong communication, and assessment skills. NJ Choice Certification is required.
Required Qualifications- Active and unrestricted RN license in New Jersey.
- Must reside in Somerset County, New Jersey.
- 3+years of clinical practice experience in settings such as hospital, home health, or ambulatory care.
- Successful completion of the NJ Choice certification (exam score 80% or higher and completion of state training modules and field mentoring).
- Reliable transportation and willingness to travel up to 75% of the time; mileage will be reimbursed per company expense reimbursement policy.
- Experience in case management and/or discharge planning.
- Experience in managed care.
- Crisis intervention skills.
Associate degree equivalent or higher.

The typical pay range for this role is $66,575.00 – $. The actual base salary offer will depend on a variety of factors including experience, education, geography, and other relevant factors. This position is eligible for a bonus, commission, or short‑term incentive program in addition to the base pay range.
