Social Work Lead
Help lead the team that connects patients to the support they need
Greater Seacoast Health Somersworth, New Hampshire | Multi-Site Support $36.91 – $41.03/hour, based on experience Full-Time | 40 Hours | Non-Exempt
Make an impact — with patients and with your team
Are you an experienced social services professional who enjoys helping people, solving problems, and supporting a team?
Greater Seacoast Health is looking for a Social Work Lead to help coordinate and support our Social Work team across our clinical locations while continuing to provide direct services to patients and families.
This is a great opportunity for someone who understands social services and is ready to take on additional responsibility, team coordination, and day-to-day leadership without stepping completely away from direct client work.
You'll help make sure our Social Workers have the coverage, communication, resources, and support they need—while working closely with our Director of Family and Social Services to keep the department running effectively.
What makes this role different?
Lead while staying connected to the work
This isn't a position where you'll spend your entire day behind a desk. You'll provide direct client support while also helping coordinate and support the Social Work team.
Be the go-to resource
Help Social Workers navigate day-to-day questions, changing priorities, coverage needs, workflows, referrals, and patient concerns.
Make an impact across Greater Seacoast Health
You'll work across our clinical sites, helping ensure patients have consistent access to social work support regardless of where they receive their care.
Help improve how we work
Partner with the Director of Family and Social Services on department projects, workflows, quality improvement initiatives, and opportunities to strengthen services.
What you'll do
As our Social Work Lead, you'll balance team coordination with direct client services. Your responsibilities will include:
- Coordinate Social Worker schedules and ensure appropriate coverage across locations.
- Provide day-to-day support and guidance to Social Workers.
- Coordinate Social Work referrals and delegate them appropriately.
- Provide direct client support, including education, referrals, and brief case management.
- Lead monthly Social Work team meetings.
- Help troubleshoot patient concerns and escalations in partnership with the Director.
- Participate in departmental projects, goals, and continuous quality improvement initiatives.
- Help identify and implement workflow improvements.
- Maintain timely, professional case documentation and help ensure documentation standards are met across the team.
- Stay current with EMR workflows and changes affecting the Social Work department.
- Represent and actively support the Social Work department throughout the organization.
- Travel between Greater Seacoast Health clinical locations as needed.
What we're looking for
Required
Bachelor's degree in Social Work or a related human services field
Excellent interpersonal and communication skills
Ability to maintain confidentiality in all circumstances
Strong computer literacy
Ability to work independently and manage multiple priorities
A client- and community-service mindset
Ability to effectively lead groups and team meetings
Strong problem-solving skills and the ability to respond effectively to both internal and external needs
Preferred
Previous supervisory or team-lead experience is preferred but not required.
We're interested in candidates who have the ability to support, organize, communicate, and lead effectively, even if their current title doesn't include "Supervisor" or "Manager."
Who will thrive in this role?
You may be a great fit if you're someone who:
- Enjoys being the person teammates can turn to for help.
- Can keep several moving pieces organized without losing sight of the people involved.
- Communicates clearly and professionally, even when situations become challenging.
- Is comfortable making decisions and solving problems.
- Can balance supporting your team with providing direct client services.
- Adapts well when schedules, priorities, or patient needs change.
- Leads through collaboration, reliability, and respect.
