Massachusetts General Hospital is seeking a Care Coordinator RN to join the Community Health initiatives within Mass General Brigham’s Clinical Community Programs. This role supports care coordination, patient outreach, referrals, and program operations across community sites and the Community Care Vans.
The RN will work with CHI Specialists, PCPs, and community partners to improve access to preventive services and manage hypertension and other priority health programs.
