Position: Clinical Team Leader (Mental Health Services)

The Team Leader for Adult Community Clinical Services (ACCS) will be responsible for providing overall administrative leadership and direction to ACCS team staff including Group Living Environments (GLE) and Supported Independent Environments (SIE) Program Directors and staff. The Team Leader oversees service delivery including intake, assessment, clinical treatment planning, and delivery, as well as peer and family support. Additionally, the Team Leader ensures seamless, high-quality team services to Persons Served living in both GLE and SIE settings while also promoting and managing successful transitions of Persons Served to other DMH services or other community programs.

The Team Leader also functions as the primary liaison to the MA Department of Mental Health (DMH), Care Coordination entities and other external parties.

Job Duties and Responsibilities
  • Provide administrative leadership, supervision and direction of Integrated Team staff, including the GLE/SIE staff. Direct the overall work activities of all Integrated Team staff and all visiting/consulting personnel while working with the team.
  • Ensure all Integrated treatment and supports adhere to the service goals of:
  • Sustained engagement and participation in services;
  • Housing in independent settings;
  • Community tenure without hospitalizations or other institutional placement;
  • Greater symptom stabilization and self-management ability, leading to self-sufficiency of PS;
  • PS satisfaction with services and personal outcomes;
  • Successful transition to other DMH or community services.
  • Function as the primary liaison with DMH, Behavioral Health Community Partners (BH CP), One Care and/or DMH Case Managers for care coordination integration.
  • Hire, supervise and evaluate all Team staff, including the Residential Director. Orient, train and coach staff of multiple disciplines, evaluate all staff for role competencies yearly, access/design training opportunities for staff as needed to enhance competencies and job performance. Foster teamwork. Arrange new staff participation in his/her respective ACCS Community of Practice Workgroup.
  • Monitor staff performance and productivity. Evaluate staff performance, including offering consistent and direct performance feedback by completing competency-based assessments, providing field supervision with Integrated Team staff, and directly observing staff while working with PS. Conduct corrective action procedures with staff if needed.
  • Coordinate intake of all referrals to Integrated Team following DMH requirements. Assign staff to orient, engage and enroll new PS.
  • Oversee service delivery to all PS, including participation in services, treatment and support skills planning and implementation of recovery goals, peer and family support, readiness for discharge, and support through service and life transitions, including acute care and housing transitions.
  • Regularly review treatment plans to ensure treatment is effective in addressing the PS needs and goals that led to enrollment, barriers to movement, or additional needs identified while enrolled in ACCS.
  • Ensure staff are trained in and use evidenced-based practices (EBP) including Motivational Interviewing;
    Harm Reduction and Addiction Treatment;
    Screening, Brief Intervention, Referral and Treatment (SBIRT);
    Housing First;
    Stages of Change;
    Seeking Safety, and Cognitive Behavioral interventions.
  • Ensure staff are trained in and employ trauma informed, recovery-oriented, person-centered, culturally competent and strengths-based approaches and skills training techniques in their work.
  • Ensure systems provide PS with Peer and Family Support, and promote the development or enhancement of natural supports and resources for PS. Work with Peer Specialists to reinforce and maintain a holistic perspective and recovery orientation for all PS.
  • Maintain systems for input from Peers and families.
  • Support the Assistant Team Leader in managing risk mitigations systems, including developing and maintaining risk alerts and procedures.
  • Ensure the seamless integration of Team and GLEs/SIEs. Assign LPHAs direct responsibility for treatment plans and services to PS in GLEs/SIEs. Co-facilitate GLE/SIE Staff Meetings on a regular basis with Residential Director.
  • Ensure adherence to Vinfen ACCS standards and EBP fidelity.
  • Ensure the timely, thorough and thoughtful documentation of PS needs and services is provided.
  • Ensure all staff receive role-based, certification- and licensure-specific supervision regularly.
  • Maintain staff schedule. Manage staff time off to maintain operational excellence.
  • Schedule LPHA Licensed staff to provide clinical coverage 24 hours/day, 7 days/week, 365 days/year, including on-call back up and on-site support and intervention when needed.
  • Maintain team interventions and systems, and the external connections needed, to promote PS health, including the management of PS medications in compliance with Medication Administration Program (MAP).
