We are currently looking for the right RN to take care of a male in Somerville. He needs 1 hour visits 12p-1p (flexible) Monday through Saturday for wound care.

• Provide high-quality, patient-centered nursing care by assessing, planning, implementing, and evaluating patient needs in the comfort of their homes.
• Collaborate closely with a multidisciplinary clinical team to optimize patient outcomes.
• Educate patients and their families on health maintenance, medication management, and disease prevention.
• Maintain accurate, timely electronic documentation in compliance with state and federal regulatory guidelines.

• License: Active, unrestricted Registered Nurse (RN) license in the State of Massachusetts.
• Experience: Previous experience in home health, hospice, or an acute care setting is highly advantageous (New grads with a passion for home health are welcome to apply!).
• Skills: Exceptional interpersonal and communication skills with a focus on empathy and clinical excellence.
• Vehicle: Reliable transportation and valid driver's license for travel to patient homes.
