We are currently looking for an RN to provide wound care to a male patient in Somerville. He requires one hourly visit between 12 p.m. and 1 p.m. (flexible) Monday through Saturday.

• Provide high‑quality, patient‑centered nursing care by assessing, planning, implementing, and evaluating patient needs in the comfort of their homes.
• Collaborate closely with a multidisciplinary clinical team to optimize patient outcomes.
• Educate patients and their families on health maintenance, medication management, and disease prevention.
• Maintain accurate, timely electronic documentation in compliance with state and federal regulatory guidelines.

• Active, unrestricted Registered Nurse (RN) license in the State of Massachusetts.
• Previous experience in home health, hospice, or an acute care setting is highly advantageous (new graduates with a passion for home health are welcome to apply).
• Exceptional interpersonal and communication skills with a focus on empathy and clinical excellence.
• Reliable transportation and a valid driver’s license for travel to patient homes.
