Position Overview

All Care is seeking a Hospice Admissions Nurse who will be responsible for evaluating patients for hospice eligibility, coordinating the admission process, and initiating high-quality, patient-centered end‑of‑life care in the home setting. This role requires strong clinical assessment skills, compassionate communication, and the ability to support patients and families during a critical transition.

Location: Somerville
Schedule: Monday‑Friday
Work Location: On the road , all service area
Position Type: Full Time

Responsibilities
  • Conduct comprehensive in‑home assessments to determine hospice eligibility in accordance with Medicare and organizational guidelines
  • Complete admissions, including clinical documentation, consents, and care planning
  • Collaborate with physicians, case managers, and interdisciplinary team members to facilitate timely admissions
  • Educate patients and families on hospice philosophy, services, and expectations
  • Develop and initiate individualized plans of care
  • Ensure accurate and timely documentation in the electronic medical record (EMR)
  • Coordinate durable medical equipment (DME), medications, and initial services
  • Provide emotional support and guidance to patients and caregivers during the admission process
  • Maintain compliance with regulatory and agency standards
  • Participate in on‑call rotation as required
Qualifications
  • Valid driver’s license and reliable transportation
  • Active Massachusetts Registered Nurse (RN) license is required
  • Minimum of 1–2 years of nursing experience; hospice, palliative care, or home health experience preferred
  • Strong clinical assessment and symptom management skills
  • Knowledge of end‑of‑life care practices
  • Excellent communication and interpersonal skills
  • Ability to work independently and manage a caseload
  • Proficiency in using electronic health record (EHR) systems for documentation and communication
