Home Health RN – Weekend Baylor

• Plan, organize, and direct home care services
• Build services from community resources to meet patient and family needs in their homes and communities
• Complete and document initial patient and family assessments
• Initiate and revise plans of care as patient status and needs change
• Administer physician-prescribed medications and treatments
• Instruct, supervise, and evaluate home health aide care every two weeks
• Identify discharge planning needs
• Maintain confidentiality under agency and Federal HIPAA guidelines
• Complete OASIS documentation
• Complete timely documentation through EPIC

• Active valid Massachusetts RN license
• Active American Health Association BLS certification
• Associate's Degree or BSN from an accredited school of nursing (BSN preferred)
• At least 1 year of direct care nursing experience
• Public health or home care nursing experience preferred
• OASIS and/or EPIC experience preferred
• Must be vaccinated against influenza as a condition of employment

Demonstrates expertise in planning and directing home care services, including patient assessments, care plan revisions, and medication administration. Proficient in OASIS documentation and maintaining HIPAA compliance while providing high-quality nursing care in home settings.
