RN Field Clinical Care Coordinator - Medford, Lexington, Waltham, MA

The RN Field Clinical Care Coordinator plays a crucial role in an Integrated Care Model by advocating for members' needs and ensuring they receive appropriate services. This position is ideal for those who are flexible, adaptable, and patient in various situations.

• Pay: $29 - $52/hour
• Location: Medford, MA
• Schedule: Full Time, Day shift

• Engage members face-to-face and/or telephonically to complete a comprehensive needs assessment.
• Assess, plan, and implement individualized care strategies.
• Identify and initiate referrals for social service programs.
• Manage the care plan throughout the continuum of care.
• Partner and collaborate with internal care teams and community resources.
• Communicate required health-related information to stakeholders.

• Education:
  • Current and unrestricted independent licensure as a Registered Nurse in the state of MA
• Experience:
  • 2+ years of clinical experience
