This is a full-time on-site role based in Somerville, NJ, for a Hospice Nurse. The Hospice Nurse will provide compassionate end-of-life care, including palliative and direct patient care, supporting patients and their families with their physical, emotional, and psychological needs. Responsibilities include conducting health assessments, administering medications, coordinating care plans, and educating patients and families about hospice services. The role requires effective communication with interdisciplinary teams and a commitment to providing dignity and comfort to all patients.
Qualifications- Expertise in Nursing, Palliative Care, and Direct Patient Care
- Experience in delivering compassionate Home Care services
- Knowledge and skills in Medicine relevant to hospice and end-of-life care
- Exceptional interpersonal, organizational, and communication skills
- Ability to work effectively within interdisciplinary teams
- Current Registered Nurse (RN) license in New Jersey
- Experience in hospice or palliative care is a strong advantage
- Valid driver’s license and reliable transportation for home visits
