Physical Therapist – Home Health and Hospice

Requirements:

• Doctor of Physical Therapy (DPT) degree or foreign equivalent.
• Physical Therapy licensure in all applicable states of practice.
• Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from an approved vendor.
• Valid Driver's License and must be at least 21 years of age.
• At least two years of recent experience in the field.

Roles & Responsibilities:

• Conduct evaluations to ensure quality standards and address specific assessment areas.
• Develop and implement individualized treatment plans for patients.
• Provide therapy interventions using appropriate treatment protocols and techniques.
• Establish and communicate individualized care and discharge plans.
• Complete comprehensive and accurate clinical documentation within established deadlines.
• Supervise assigned assistants.
