We are seeking a skilled and compassionate Wound Care NP or PA to join a mobile wound care team providing services within skilled nursing facilities in the Sonora, Merced, and San Andreas areas. This is a weekday‑only opportunity with flexible scheduling and strong earning potential.

AboutThe Role
This provider will evaluate and manage acute and chronic wounds in collaboration with facility nursing staff and interdisciplinary teams.
Wound Types Include- Pressure injuries (all stages)
- Diabetic foot ulcers
- Venous stasis ulcers
- Post‑surgical wounds
- Skin tears, rashes, and malignant wounds
This is a mobile position, and travel between assigned facilities may be required.
Schedule- Weekday daytime hours (morning‑heavy schedule)
- No weekend shifts
- Flexible part‑time or expanded schedule options
- Hours vary based on patient caseload
- Opportunity to cover one or multiple facilities
Current need is primarily part‑time, with potential to grow.
Responsibilities- Perform comprehensive wound assessments and develop care plans
- Order and interpret diagnostics
- Prescribe appropriate treatments and topical therapies
- Perform conservative sharp debridement
- Manage negative pressure wound therapy
- Document wound progress with photography and detailed notes
- Educate patients and caregivers on wound care and prevention
- Collaborate with physicians, nurses, therapists, and care teams
- Ensure compliance with CMS and regulatory standards
- Master’s degree from an accredited NP or PA program
- Active, unrestricted California NP or PA license
- National board certification
- Active NPI and DEA (or eligibility)
- Wound care, dermatology, or surgical experience
- Wound care certification (WCC, CWCN, CWS) preferred
- Experience with electronic health records
- Base Salary + RVU/Productivity Incentive
- Annual Compensation: $40,000 - $160,000 (based on caseload and schedule)
