Join our healthcare partner as a Care Manager Registered Nurse, where you'll play a vital role in coordinating and delivering exceptional home health services. Your expertise will ensure that patients receive the highest quality care, while you enjoy the flexibility and autonomy that comes with working in a home health setting.
- Coordinate and manage patient care plans in collaboration with interdisciplinary teams
- Conduct comprehensive assessments to determine patient needs and develop individualized care plans
- Provide education and support to patients and their families to promote optimal health outcomes
- Ensure compliance with all regulatory and accreditation standards
- Maintain accurate and timely documentation of patient care and progress
- Registered Nurse (RN) licensure in the state of practice:
Required - BLS certification:
Required - Valid Driver's License and must be at least 21 years of age or older:
Required - Current automobile insurance and a reliable automobile:
Required - Need 2 years experience in setting
