Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from an approved vendor
Must-Have:
Valid Driver's License and must be at least 21 years of age or older; current automobile insurance and a reliable automobile

The RN Care Manager plans, organizes, and directs home care services to meet the needs of patients in their homes.
This role includes managing a patient caseload in an effective and efficient manner.
The RN provides patient care independently, collaborating with other core disciplines to meet care goals.
They function as a resource to patients, family members, and other care team members.

Essential Functions:

Completes initial and ongoing comprehensive assessments of patient needs.
Conducts nursing assessments of assigned patients and discusses aspects of nursing care.
Identifies and reports problems, abnormal findings, or nursing diagnoses that require follow-up.
Implements the plan of care in accordance with identified needs.
Participates with the patient in clarifying mutually agreed-upon goals.
Provides direct and/or indirect skilled, safe, and quality care.
Coordinates patient care with other disciplines and integrates input from those disciplines.
Performs ongoing nursing assessments and modifies plans of care as needed.
Documents patient care delivery, patient status, responses to treatment, and therapeutic interventions in accordance with established policies.
Ensures timely completion and submission of all documentation.
Updates nursing plans and medication profiles when changes occur.
Counsels and educates patients and families in meeting healthcare and nursing needs.
Provides learning opportunities for patients and caregivers, including written materials as appropriate.
Initiates appropriate preventative and rehabilitative nursing procedures.

Complies with all applicable legal requirements, standards, policies, and procedures.
Demonstrates professional conduct, service excellence, and commitment to organizational mission and values.
On-call duties may be assigned based on operational needs.
Performs other job-related duties as assigned.
