Summary: The purpose of this position is to provide quality, comprehensive nursing services to Tribal individuals and families and to oversee nursing services at the Tuolumne and Sonora Medical Clinics. The role focuses on prevention of illness and promotion of health of high‑risk tribal members through outreach, follow‑up, scheduling visits, coordinating care and medication management, and providing therapeutic and educational services.

• Act as the secondary Infection Control Lead for all clinical locations.
• Review and revise Tribal Member Well‑Being Department policies, procedures, objectives, and work plans annually.
• Develop short‑ and long‑range plans for improving community health standards with the Chief Medi‑Cal Director and/or Director of Nursing; incorporate evaluation tools to assess current standards.
• Develop and implement staff in‑service training on a regularly scheduled basis.
• Develop and submit proposals for funding of Tribal Member Well‑Being Department objectives.
• Coordinate the collection and tabulation of data representing all monthly staff activities.
• Establish and maintain a tracking system to ensure timely action on referrals to and from the Department.
• Oversee LVN Clinic Nurse Manager, Clinic Service Manager, Enhanced Care Management Coordinator, Community Health Worker, and other employees as needed.
• Assist in orientation and training for new Tribal Member Well‑Being staff.
• Familiarize with the aims and objectives of Health and Well‑Being for the Tribal community.
• Perform or coordinate home skilled nursing procedures and treatments, teaching Native American individuals and families to care for themselves.
• Utilize the nursing process (assessment, diagnosis, planning, intervention, evaluation) to ensure care plans reflect current health conditions and interventions.
• Identify and track hospitalized qualified Native American patients and provide follow‑up home care and health education.
• Receive interdepartmental and outside health agency referrals for home visits to acute and chronic care patients.
• Provide health education on chronic disease management (diabetes, hypertension, women’s and men’s health, substance abuse, communicable disease control).
• Maintain an immunization program for all registered TMWIHC Native American Indian patients in compliance with state, federal, and IHS guidelines.
• Contribute program notes and care plan updates to Medi‑Cal records for each client served by Outreach staff.
• Ensure charting complies with IHS regulations or guidelines.
• Maintain a high‑risk client list for each area and coordinate care with TMWIHC primary care providers.
• Attend regularly and consistently; interact professionally and appropriately with others.
• Work collaboratively with Elder Home Care and Elder program staff.
• Perform other duties as assigned.

• Graduate of an accredited school of nursing and current Registered Nurse (RN).
• Current Provider Basic Life Support (BLS) card.
• Minimum two years of manager‑level experience in a clinical setting or equivalent experience and education.

• Knowledgeable of chronic conditions.
• Ability to observe and react to adverse health indicators and respond appropriately in emergency situations.
• Strong relationship‑building skills with Tribal members to gain trust and confidence.
• Ability to plan, assign and direct work of others.
• Effective oral and written communication skills; ability to follow instructions.
• Independently self‑directed with initiative; accepts supervision and follows directions.
• Proficiency with computers, office equipment, and Electronic Health Records.
• Ability to maintain confidentiality and handle sensitive matters with discretion.
• Knowledge of HIPAA privacy requirements.
• Ability to work in a fast‑paced and sometimes stressful environment.
• Willingness to travel as needed.
• Successfully pass a comprehensive background investigation with no felony convictions.
