Duration: 13 weeks (possible extension)

Work Schedule:

8 hours Days. (8:00 am – 4:30 PM)

** Nurse will also receive reimbursement for miles driven while performing job duties, such as traveling to patient homes in a home health role.**

Responsibilities for this position include, but are not limited to:

• Conduct comprehensive patient assessments and develop individualized care plans.
• Coordinate care with physicians, therapists, social workers, and other healthcare providers.
• Monitor patient progress and adjust care plans as needed.
• Provide patient and family education regarding disease management, medications, and treatment plans.
• Ensure accurate and timely documentation of patient care activities.
• Advocate for patients and assist in identifying community resources and support services.
• Maintain compliance with home health regulations, organizational policies, and quality standards.

To be considered for this position, you should have:

• Active Registered Nurse (RN) license in the state of practice – Required.
• CPR or Basic Life Support (BLS) certification from an approved vendor – Required.
• Valid Driver's License (DL) - Required.
• Current automobile insurance and a reliable automobile – Required.
• Previous Home Health or Care Management experience preferred.
• Strong assessment, care coordination, communication, and time-management skills.
