Short-term acute care hospital-based hospice program - Mileage reimbursement provided in addition to standard pay
- Conduct initial and ongoing assessments of patients' physical, emotional, psychosocial, and spiritual needs; develop and update individualized care plans with the interdisciplinary team
- Deliver skilled nursing care, including medication management, pain and symptom control, wound care, and patient/family education on disease process and hospice philosophy
- Provide emotional support and crisis intervention to patients and families as needed
- Serve as the primary case manager for an assigned caseload (typically 12-18 patients), ensuring continuity of care
- Communicate regularly with physicians, social workers, chaplains, hospice aides, and other team members; participate in interdisciplinary team meetings
- Document accurately in the electronic medical record (EMR) in compliance with regulatory requirements; ensure compliance with Medicare/Medicaid and state/federal hospice regulations
- Participate in the on-call rotation and respond promptly to after-hours patient needs
- Nursing license:
Registered Nurse (RN) license in the state of practice (California) - Required - Certifications:
Current CPR or Basic Life Support (BLS) certification from an approved vendor
- Required - Driver's license and auto insurance:
Valid driver's license, at least 21 years old, current automobile insurance
- Required - Experience:
At least two years of recent nursing experience in the field; hospice experience required for travel assignment - Education:
BSN preferred - Ability to work in a hospital-based hospice setting and to coordinate care across disciplines
- Guaranteed hours on assignment
- Mileage reimbursement for travel
- Dedicated travel support and ongoing recruiter assistance
- Credentialing and onboarding assistance
- On-call and after-hours support provided as part of the program
