• Mileage reimbursement provided in addition to standard pay

• Conduct initial and ongoing assessments of patients' physical, emotional, psychosocial, and spiritual needs; develop and update individualized care plans with the interdisciplinary team
• Deliver skilled nursing care, including medication management, pain and symptom control, wound care, and patient/family education on disease process and hospice philosophy
• Provide emotional support and crisis intervention to patients and families as needed
• Serve as the primary case manager for an assigned caseload (typically 12-18 patients), ensuring continuity of care
• Communicate regularly with physicians, social workers, chaplains, hospice aides, and other team members; participate in interdisciplinary team meetings
• Document accurately in the electronic medical record (EMR) in compliance with regulatory requirements; ensure compliance with Medicare/Medicaid and state/federal hospice regulations
• Participate in the on-call rotation and respond promptly to after-hours patient needs

• Nursing license:
  Registered Nurse (RN) license in the state of practice (California) - Required
• Certifications:
  
  Current CPR or Basic Life Support (BLS) certification from an approved vendor
  - Required
• Driver's license and auto insurance:
  Valid driver's license, at least 21 years old, current automobile insurance
  - Required
• Experience:
  
  At least two years of recent nursing experience in the field; hospice experience required for travel assignment
• Education:
  
  BSN preferred
• Ability to work in a hospital-based hospice setting and to coordinate care across disciplines
