• Setting: 100% Inpatient

• Call Type: 24-Hour Call

• Patient Volume: 8 10 hospital-based patients per day

• Adult Population: 100%

• Other Neurologists Onsite During Coverage: No

• Neurology Group Size: 6 MDs (IP/OP combined)

• APP Support: 1

• EMR: Epic

• Lab & X-Ray: Onsite

• Board Certified in Neurology

• Active Indiana License
  - Can work with IMLC candidates

• Self-Query NPDB report (pulled within same month as submission)
