Overview
Majestic Care of South Bend is looking for a Physical Therapist to join our team's mission and believe in our core values! Our mission:

- Conduct initial and ongoing evaluations to assess residents’ physical therapy needs, including mobility, strength, balance, and functional limitations.
- Develop and implement individualized treatment plans based on clinical findings, resident goals, and functional abilities.
- Provide direct physical therapy interventions such as therapeutic exercises, gait and balance training, mobility enhancement, and manual therapy.
- Utilize evidence-based practices to promote optimal functional outcomes and independence.
- Monitor and document residents’ progress, adjusting treatment plans as necessary to reflect changes in condition or goals.
- Educate residents, families, and caregivers on treatment plans, home exercises, and safety strategies to support continued progress and prevent injury.
- Collaborate with interdisciplinary team members—including nursing, occupational therapy, and speech therapy—to ensure coordinated, resident-centered care.
- Maintain accurate and timely documentation of all evaluations, treatments, progress notes, and discharge planning in accordance with SNF, state, and federal guidelines.
- Ensure compliance with all regulatory and facility policies, including HIPAA, OSHA, infection control protocols, and Medicare PDPM requirements.
- Participate in quality improvement initiatives and contribute to the development of best practices within the rehabilitation department.
- Stay current with advancements in physical therapy, geriatric rehabilitation, and regulatory standards.
- Engage in continuing education and professional development opportunities to enhance clinical skills and knowledge.
- Provide training and mentorship to physical therapy assistants and other team members to support clinical excellence and consistency of care.
- Oversee and support physical therapy assistants (PTAs) in accordance with state practice acts and facility guidelines.
- Provide coaching, development, and corrective action as needed to ensure high-quality service delivery.
- Doctorate or Master’s degree in Physical Therapy from an accredited program.
