This is a part-time position. Day shift.
Primary duties:
- Help clients gain access to needed medical, social, educational and other services.
- Provide direct assistance in gaining access to services, coordination of care, oversight of the entire case and linkage to appropriate services.
Areas of service:
- Assessment of the eligible individual to determine housing needs
- Responsible for accurate and timely documentation related to grant deliverables
- Referral and related activities to advocate and assist the individual obtain needed services
- Monitoring and follow up of the housing plan
- Linkage and networking with court, health care providers, employers and other community agencies
- Provide integration and assistance to clients and families to help sustain within the community/least restrictive environment
- Accountable for the flow of information within a multidisciplinary team setting
- Special Populations/Skills/Employment duties, as assigned.
- High School Diploma / GED required; BA/BS preferred.
- Experience with targeted population highly preferred.
- Valid driver's license
- Proof of insurance
