• Conduct evaluations and develop individualized, goal-oriented treatment plans
• Deliver hands‑on, evidence‑based therapy focused on restoring function and reducing pain
• Educate and motivate patients and families to support home progress
• Collaborate with referring providers and rehab team for optimal outcomes
• Supervise therapy assistants and mentor students when applicable

• Bachelor's, Master's, or Doctorate in Physical Therapy
• Active or eligible PT license in the State of Indiana
• Current CPR/BLS certification
• Strong communication, clinical, and time management skills
• Experience in outpatient care preferred
