Hospital Discharge Coordinator reports to the specialty BMG manager/director and is responsible for coordinating specialty follow‑up care for patients following hospital and emergency department discharge. The role schedules specialist appointments, verifies insurance eligibility, communicates coverage and financial responsibility to patients, and collects applicable co‑payments and deductibles. This position supports continuity of care and access to specialty services on behalf of the medical group.

- Receive and greet patients and visitors when checking out after emergency services.
- Obtain patient information to accurately complete the patient record for payment information and scheduling purposes.
- Maintain good rapport and a cooperative working relationship with all members of the medical offices and hospital departments.
- Act as liaison between provider offices and hospital departments to ensure timely access to follow‑up care and represent Beacon Medical Group.
- Collect copays and deductibles at discharge to support health system financial goals while maintaining strong patient relations.
- Post patient payments on the patient’s account, generate a system receipt, and give it to the patient.
- Perform online insurance eligibility checks and specialty referral scheduling.
- Gather pertinent information from insurance carriers and ancillary staff to determine the patient’s financial obligations for services rendered.
- Refer patients for financial assistance if they cannot secure satisfactory payment arrangements.
- Document communications, actions, and other data about referrals in the information system.
- Evaluate current processes for enhancement and make necessary recommendations.
- Run emergency department discharge reports to identify patients discharged without specialist referral coverage; proactively contact identified patients to schedule appropriate specialist follow‑up.
- Assist management in identifying referral patterns by inputting referral data into the referral database.
- Communicate with the Manager (or Director) regarding any concerns or problems.
- Attend and participate in department meetings, sharing all necessary information.
- Complete mandatory education, annual competencies, and department‑specific education within established time frames.
- Complete annual employee health requirements on time.
- Maintain current BCLS (CPR) and other required certifications.
- Consistently use appropriate universal precautions, protective equipment, and ergonomic techniques.
- Adhere to regulatory agency requirements, survey process, and compliance.
- Comply with established organization and department policies.
- Be available to work overtime and additional shifts as required.

- High school diploma or equivalent (required).
- Minimum age 17.
- Preferred: minimum one year of previous hospital or medical office experience; associate’s degree preferred.
- Desired: additional college‑level courses in the medical practices area.
- Basic office and keyboarding skills; ability to use designated reference materials and office equipment.
- Effective telephone skills for accurate data collection and communication.
- Proficient computer skills (data entry, word processing, spreadsheets); ability to use multiple databases such as Availity, Carelon, and Cerner.
- Understanding of time‑of‑service collections, including co‑pays and deductible insurance requirements.
- Analytical skills to solve patient problems and interpret data.
- Tactful handling of highly personal and confidential patient issues.
- Promotion of collaboration and innovation in clinical services.
- Professionalism in frustrating interpersonal situations.
- Hours are subject to change based on departmental or organizational needs.
- May need to travel to other Beacon locations.
