Job Overview

RWJBarnabas Health, Primary Care Service Live is seeking a Full‑Time Advanced Practice Provider to provide primary care coverage in Southern NJ. The role involves clinical care, telemedicine, and regional onsite support.

    • CDS and DEA
    • BLS
    • Master’s degree required or Graduate Level Certification

Essential Functions
  • Performs comprehensive initial and episodic health assessments to identify existing or potential health problems.
  • Initiates and implements individualized nursing and medical plans of care in collaboration with interdisciplinary team members.
  • Initiates appropriate diagnostic, therapeutic, medical, and nursing actions in conjunction with physicians.
  • Provides follow‑up care including acute and routine examinations.
  • Assesses progress, identifies problems, initiates preventive measures, recognizes emergent problems, and initiates proper care in collaboration with physicians.
  • Monitors and adjusts patient medication according to protocol or medical plan of care.
  • Implements nutritional therapies as appropriate.
  • Triages patient problem calls requiring medical attention.
  • Orders appropriate laboratory and diagnostic procedures.
  • Recognizes normal and abnormal laboratory findings and the associated pathology.
  • Documents information in patient charts following documentation standards.
  • Provides health education to patients and families (e.g., management of side effects, anticipatory teaching).
  • Manages medical emergencies as indicated, according to standing orders.
  • Provides monitoring and continuity of care between healthcare settings.
  • Provides patient and family counseling and reassurance regarding psychological and psychosocial issues.
  • Initiates proper referrals to private or community agencies.
  • Acts as a resource and teacher for other members of the healthcare team.
  • Shares knowledge and expertise with staff.
  • Collaborates with nursing and medical staff at partner and affiliate sites to coordinate continuity of care.
  • Evaluates the quality of care and quality of life for patients through assessment of patient responses and recommends interventions as indicated.
Travel Requirement
  • Provides both onsite and telemedicine support and coverage for practices as needed.
  • Provides coverage for all practice sites within the assigned regional territory.
  • Mileage reimbursement will be provided for travel from the primary department to assigned locations.
  • Frequency of travel will be contingent on practice needs at the assigned location.
  • Duration of coverage will be contingent on practice needs at the assigned location.
