Home Health Speech Language Pathologist - per visit

Overview

BAYADA Home Health Care is seeking a Home Health Speech Language Pathologist for a role in South Burlington, Vermont. This is a full-time, part-time, or per diem opportunity performing home health visits.

Office location:
Williston, Vermont. This office serves adult and geriatric patients in homes throughout Chittenden County. Prior home care experience as an SLP is a plus, but not required.

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations to assess, diagnose, and provide therapeutic interventions for speech, language, communication, or swallowing difficulties.
• Develop a treatment plan based on the evaluation, physician’s orders, and recommended goals.
• Continuously assess and revise the speech therapy care plan and participate with BAYADA clinicians in the multidisciplinary care plan as appropriate.
• Educate and instruct patients, family members, or other representatives in rehabilitative care to promote health, safety, and independent living.
• Accurately document observations, interventions, and evaluations pertaining to patient care using the organization’s EMR tools.

• Current license as a Speech Language Pathologist in Vermont.
• Certificate of Clinical Competence in Speech from the American Speech-Language-Hearing Association (ASHA CCC), or equivalent educational requirements and work experience for certification.
• Alternatively, eligibility or ongoing process toward ASHA certification as indicated by transcripts or diploma.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
