Position: Registered Nurse (RN) - Home Health - $60+ per visit

BAYADA Home Health Care is seeking a Registered Nurse (RN) for home health visits in South Burlington, Vermont. This staff, ongoing position offers flexible schedules, 1:1 care, and a strong work/life balance.

Job Description & Requirements

Responsibilities
  • Follow a designated care plan in accordance with patients' needs.
  • Make home visits to clients within the assigned geographic territory.
  • Perform assigned duties, including administration of medication, wound care, treatments, and procedures.
  • Monitor clients' conditions and report changes to the Clinical or Client Services Manager.
  • Follow up with, execute, and properly document doctors' orders.
  • Perform client assessments as necessary.
  • Engage in case management and coordination of care.
  • Accurately document observations, interventions, and evaluations related to client care using a state-of-the-art tablet.
Qualifications
  • A current license as a Registered Nurse in Vermont.
  • Minimum of one year of recent, verifiable clinical (medical/surgical) experience.
  • Prior home care experience strongly preferred, but not required.
  • Graduation from an accredited and approved nursing program, verified by transcript or diploma.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal and communication skills.
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred.
  • Ability to travel to cases as assigned.
