BAYADA Home Health Care is seeking a Registered Nurse (RN) for home health visits in South Burlington, Vermont. This staff, ongoing position offers flexible schedules, 1:1 care, and a strong work/life balance.
Job Description & Requirements
Responsibilities- Follow a designated care plan in accordance with patients' needs.
- Make home visits to clients within the assigned geographic territory.
- Perform assigned duties, including administration of medication, wound care, treatments, and procedures.
- Monitor clients' conditions and report changes to the Clinical or Client Services Manager.
- Follow up with, execute, and properly document doctors' orders.
- Perform client assessments as necessary.
- Engage in case management and coordination of care.
- Accurately document observations, interventions, and evaluations related to client care using a state-of-the-art tablet.
- A current license as a Registered Nurse in Vermont.
- Minimum of one year of recent, verifiable clinical (medical/surgical) experience.
- Prior home care experience strongly preferred, but not required.
- Graduation from an accredited and approved nursing program, verified by transcript or diploma.
- Ability to work independently and manage time effectively.
- Strong interpersonal and communication skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
- Ability to travel to cases as assigned.
