• Provide direct nursing care to residents per established care plans
• Administer medications and document treatments according to policy
• Monitor resident status, identify changes, and communicate with the supervising nurse
• Assist with activities of daily living and mobility
• Collaborate with interdisciplinary team, including physicians and therapists
• Ensure safety, infection control, and accurate documentation

• Active LPN license (state-specific requirements apply)
• Ability to work in a long-term care environment
• Availability to work 40 hours per week across Day and/or Evening shifts
• Willingness to participate in a travel assignment
• Strong communication and teamwork skills
