• Administer prescribed medications and treatments in accordance with physician orders
• Monitor residents' conditions, perform routine nursing assessments, and document vital signs
• Provide wound care, injections, and other clinical tasks as delegated by the RN or physician
• Assist residents with activities of daily living, ensuring dignity, safety, and comfort
• Maintain accurate charting and communicate changes to the supervising RN/physician
• Adhere to infection control and safety protocols; maintain a clean, safe care environment

• Active LPN license
• Current CPR/BLS certification
• Ability to work evenings and/or overnights in a long-term care setting
• Strong documentation and communication skills
• Ability to pass background checks and health screenings
