• Travel temporary assignment in a long-term care setting

• Provide direct nursing care to residents in a long-term care setting, including monitoring vitals, administering medications and treatments per physician orders, and assisting with activities of daily living
• Perform ongoing assessments and document changes in resident conditions accurately in the charting system
• Collaborate with the interdisciplinary team (RN, physicians, therapists, social workers) to implement and adjust individualized care plans
• Recognize and report acute changes in residents’ conditions promptly to the supervising nurse and physician as needed
• Ensure adherence to infection control, safety, and privacy policies; maintain clean, safe, and organized units
• Respond to call bells, participate in rounds, and provide compassionate, resident-centered care

• Active Licensed Practical Nurse license in Vermont
• CPR/BLS certification
• Ability to work Evening and/or Overnight shifts
• Strong communication and documentation skills
• Willingness to participate in a travel assignment in a long-term care setting
• Ability to maintain professionalism and teamwork in a dynamic care environment
