Licensed Practical Nurse (LPN) – Assisted Living in a memory care setting located in South Burlington, Vermont. • Provide direct clinical care to memory care residents, including monitoring comfort, mobility, and safety.
• Administer medications and treatments per physician orders; document care in the resident chart.
• Monitor vital signs, notice changes in condition, and report promptly to the supervising nurse.
• Assist residents with activities of daily living (ADLs) and support mobility, hydration, nutrition, and hygiene.
• Collaborate with the interdisciplinary team to implement and update individualized care plans; communicate with families as appropriate.
• Maintain accurate documentation, follow infection control practices, and ensure a safe, respectful environment for residents.

• Active Licensed Practical Nurse (LPN) license or ability to obtain licensure for the appropriate jurisdiction
• Graduate of an approved practical nursing program
• Prior experience in long-term care or memory care settings preferred
• Ability to work night shift and maintain reliability and flexibility
• Strong communication, assessment, and documentation skills
• Must be able to pass background check and drug screening
