This is a full-time, on-site role based in Columbia, SC, for an Advanced Practice Provider, Nurse Practitioner, or Physician Assistant. The role involves evaluating and managing patients with nephrology and hypertension conditions, conducting patient assessments, and developing individualized treatment plans in collaboration with the healthcare team. Additional responsibilities include educating patients about disease management, participating in quality improvement initiatives, and maintaining accurate medical records.

• Proficiency in patient assessment, diagnosis, and management of nephrology and hypertension conditions
• Strong knowledge of nephrology practices, transplantation procedures, and apheresis treatment
• Skills in patient education, communication, and developing treatment plans tailored to individual needs
• Experience with electronic medical records (EMR) systems and maintaining accurate documentation
• Ability to collaborate effectively with multi-disciplinary teams and provide compassionate, patient-centered care
• Relevant certification and licensure as a Nurse Practitioner (NP) or Physician Assistant (PA) with the ability to practice in South Carolina
• Prior experience in nephrology or a related field is highly desirable
