Early Intervention Services (EIS) – Medical Case Manager POSITION SUMMARY
Little River Medical Center's EIS team aims to provide access to health care services and medication for people diagnosed with HIV/AIDS and to strengthen skills to achieve optimal treatment adherence with the goals of viral suppression and reduced HIV-related mortality.
The EIS Medical Case Manager provides comprehensive, patient-centered medical case management services to people living with HIV (PLWH) served by Little River Medical Center (LRMC). Services are delivered in accordance with the Ryan White HIV/AIDS Program (RWHAP) and HRSA/HABguidelines andare aligned with Patient-Centered Medical Home (PCMH) principles and national Ending the HIV Epidemic (EHE) priorities.
The position supports equitable access to HIV primary care, sustained engagement in treatment, viral suppression, and improved health outcomes through coordinated medical, behavioral health, and supportive services. Care is delivered using trauma-informed, culturally responsive, and nondiscriminatory practices that advance health equity and reduce barriers to care for historically underserved and marginalized populations.

ESSENTIAL DUTIES AND RESPONSIBILITIESinclude the following. Other duties may be assigned:
Medical Case Management & Care Coordination- Provide Ryan White–compliant medical case management services, including intake, eligibility determination, comprehensive assessments, individualized service planning, ongoing reassessment, and care coordination.
- Coordinate HIV primary care, specialty care, behavioral health, substance use services, and supportive services based on patients’ needs and acuity.
- Facilitate referrals and warm handoff to LRMC PCPs, dental and behavioral health, as well as external community-based providers consistent with PCMH care coordination standards.
- Support patients with appointment scheduling, medication adherence, health literacy, and navigation of healthcare and social service systems.
- Promote continuous engagement in HIV medical care and adherence to treatment plans to support viral suppression and improved health outcomes.
- Conduct outreach, follow-up, and re‑engagement for patients who are newly diagnosed, out of care, or at risk of disengagement.
- Support EHE‑aligned strategies by addressing structural and social barriers that impact access, retention, and equity in HIV care.
- Function as an integral member of the interdisciplinary care team within LRMC’s PCMH model.
- Collaborate with primary care providers, nurses, behavioral health clinicians, pharmacists, outreach staff, and community partners to deliver coordinated, whole‑person care.
- Participate in interdisciplinary case conferences, care coordination meetings, and population health management activities.
- Support team‑based workflows that emphasize access, continuity, coordination, quality, and patient‑centeredness.
- Maintain accurate, timely, and compliant documentation in the electronic health record (EHR) and Ryan White data systems.
- Ensure compliance with Ryan White HIV/AIDS Program requirements, HRSA/HAB policies, eligibility standards, and scope‑of‑project guidelines.
- Support data collection and reporting activities, including Ryan White Services Report (RSR), UDS‑aligned HIV quality measures, and internal quality dashboards.
- Participate in Ryan White Clinical Quality Management (CQM) and Continuous Quality Improvement (CQI) activities.
- Assist with HRSA and State DPH site visits, audits, and program monitoring activities.
- Advocate for patients to address barriers related to stigma, discrimination, social determinants of health, and healthcare access.
