  • Licensure: LSW, LBSW or LMSW
  • Experience:

    Prior work experience in a community or long-term care setting is preferred
  • Skills:

    Communication, assessment, social work methods, and techniques. Ability to work effectively with various disciplines in individual and team settings, understand social systems, implement state and federal regulations, and work in a fast‑paced capacity.
Duties/Responsibilities

Clinical:

  • Complete a social history and psychosocial assessment for each resident that identifies social, emotional, and psychological needs.
  • Work with individual patients and families toward adjustment to center life and to crises of illness, disability, and death.
  • Serve in an advocacy role for patients and their families in expressing, defining, and resolving grievances.
  • Report to the Director of Social Work Services the current status of social services and present needs.
  • Participate in developing a written, interdisciplinary plan of care for each resident that identifies the resident's psychosocial needs/issues, goals to be accomplished, and appropriate social worker interventions.
  • Ensure or provide therapeutic interventions to assist residents in coping with their transition and adjustment to a long‑term care facility, including their social, emotional, and psychological needs.
  • Provide support and education to residents/family members/significant others to assist in understanding placement and facility issues and refer them to the appropriate social service agencies when the facility does not provide the needed services.
  • Provide clinical interventions to address catastrophic events during the resident's stay in the facility.
  • Coordinate the resident discharge planning process and make referrals for appropriate home care services before the resident's return to the community.
  • Maintain confidentiality of professional information.

Administrative:

  • Develop, maintain, and utilize a listing of current community resources helpful to residents and their families and significant others.
  • Participate in quality assurance interdisciplinary team meetings.
  • Understand and meet all government requirements for social service documentation.
  • Document progress in meeting the psychosocial needs of residents.

Advocacy:

  • Work with the interdisciplinary team and administration to promote and protect resident rights and psychological well‑being. Prevent and address resident abuse as mandated by law and professional licensure.
  • Identify community changes and opportunities such as legislation, regulations, and programs that affect nursing home residents.
  • Work with residents, families, significant others, and staff to provide support, information, and organization for proactive self‑advocacy to improve quality of life/care.
Experience and Knowledge
  • Prefer Bachelor's degree from an accredited college or university.
  • Must know the medical case management process, needs of geriatric patients, health care settings, and funding resources such as Medicare Part A, B, C, D, Medicaid, Managed Care, and other private insurance.
  • Must have imagination, creative ability, skill in working cooperatively with other professionals, and ability to organize and carry out responsibilities efficiently and effectively.
  • Medical knowledge and terminology preferred.
  • Marketing experience preferred.
  • Ability to interact in person with potential admissions in a hospital setting.
  • Multitasking is a must.
  • Commitment to the mission and goals of the center.
  • Ability to exercise independent judgment where procedures cannot be standardized.
