A company is looking for a Health Coach (NBC-HWC).
Key Responsibilities Provide one-on-one coaching to members managing or at risk for chronic health conditions Use evidence-based guidelines to educate and empower members in achieving health goals Partner with healthcare teams to improve member experience and outcomes while reducing costs
Required Qualifications , Training, and Education Associate degree, preferably in a healthcare-related field Completion of a training program approved by the National Board for Health and Wellness Coaching (NBHWC) At least one year of experience in a health coaching-related field preferred
Experience with chronic conditions such as diabetes, obesity, and hypertension is preferred Solid understanding of health behavior change theories and their practical application
