Carlyle Senior Care of Blackville is seeking a compassionate, organized, and motivated Admissions / Social Services Coordinator to join our team! Our facility provides both short-term rehabilitation and long-term care services, and we are committed to delivering exceptional care and support to our residents and families.

The Admissions / Social Services Coordinator plays an important role in welcoming new residents, assisting families through the admission process, coordinating discharge planning, and supporting the emotional and social well-being of our residents.

• Coordinate admissions and resident transitions into the facility
• Communicate with hospitals, referral sources, residents, and family members
• Assist residents and families with social service needs and discharge planning
• Maintain accurate admission and social services documentation
• Help ensure resident rights and quality of life are upheld
• Build positive relationships with residents, families, and interdisciplinary team members
• Assist with care planning and resident advocacy

• Previous experience in healthcare admissions, social services, case management, or long-term care preferred
• Excellent communication and organizational skills
• Compassionate and resident-centered approach
• Ability to multitask and work in a fast-paced environment
• Knowledge of Medicare, Medicaid, and long-term care processes is a plus
