Physical Therapist – Skilled Nursing / Long-Term Care

$45 - $55 per hour | Marion, SC | On-site | Permanent

We are a skilled nursing and long-term care organization focused on providing compassionate, resident-centered care. Our in-house rehabilitation team works closely with nursing, physicians, families, and other disciplines to help residents regain function, maintain independence, and achieve meaningful goals.

• Evaluate mobility, strength, balance, endurance, and functional abilities
• Develop individualized physical therapy treatment plans
• Provide therapeutic exercise, gait training, balance work, transfer training, and functional mobility treatment
• Help residents recover following illness, injury, surgery, or hospitalization
• Identify fall risk, weakness, and changes in functional status
• Educate residents, families, and caregivers
• Collaborate with nursing, physicians, OT, SLP, and other members of the care team
• Monitor progress and modify treatment plans as appropriate
• Complete accurate and timely clinical documentation

• Current or eligible South Carolina Physical Therapist license
• CPR certification
• Strong clinical assessment and treatment-planning skills
• Ability to work with geriatric and medically complex patients
• Strong communication and interdisciplinary teamwork skills
• Compassionate, patient-centered approach
