• Assist with personal care and activities of daily living
• Support safe mobility, transfers and independence
• Prepare meals and provide light household support as assigned
• Offer companionship and respectful emotional support
• Follow the care plan and report changes or concerns promptly
• Communicate professionally with clients, families and the Home Well team
