Your Health Organization in South Carolina is seeking a dedicated Community Health Worker (CHW) to service patients in the Kingstree, Effingham, Pamplico area. The CHW will visit patients at homes and ALFs, coordinate visits with providers and pharmacies, and deliver medications and DME to ensure timely access to care.
Travel is required daily with reliable transportation. This full-time, salary-based role involves 12-hour shifts from 7:00 AM to 7:00 PM.
