Licensed Practical Nurse, Home Health – Center Well Home Health
Humana invites qualified Licensed Practical Nurses (LPN) to join Center Well Home Health. You will provide personalized, one‑on‑one care to patients in their homes, working closely with physicians and the care team to develop and manage individualized care plans.
Responsibilities- Provide professional nursing services under RN direction in compliance with state regulations and Center Well policies.
- Deliver skilled nursing interventions for patient treatment, rehabilitation, and preventive care, using a holistic approach.
- Apply clinical standards to facilitate problem resolution and achieve patient goals.
- Consult with RN supervisor regarding changes in treatment plans and communicate physician orders, when permitted.
- Implement goal‑driven nursing plans and maintain documentation.
- Coordinate with the healthcare team and communicate patient/family needs promptly.
- Maintain professional conduct and confidentiality of patient information.
- Ensure supplies, materials, and equipment are available for safe care delivery.
- Prepare, submit, and maintain documentation including shift notes; document day services rendered.
- Graduate of an accredited Licensed Practical Nursing Program or Vocational Nursing School.
- Current LPN license in the practicing state.
- Valid driver’s license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting, preferably home health or hospice.
• $28.00 – $40.00 per visit/unit
• $44,600 – $61,400 per year base pay
