The Cardiac Navigator position serves as a key contact for cardiac medical patients. Responsible for ensuring the cardiac patient population served has safe, high quality and efficient care throughout the continuum of care. Monitors to assure all quality benchmarks are reached with follow up education to staff as indicated. Assists with analysis and action plans for concurrent database monitoring. Role also acts as a resource internally and in the community to educate the cardiac medical/heart failure population.

Cardiac Navigator will aid in facilitating evidence-based order sets for these populations. Will be accountable for collaborating with members of the health care team to design, measure and implement safe, cost-effective, evidence-based care strategies. The Cardiac Navigator will work collaboratively with the ACS Patient Navigator and Heart Failure Navigator, maintaining current professional knowledge and competencies and contributing to the advancement of the practice of cardiac treatments nursing at the unit/system, local, state and/or national level.
