Job Summary
We are seeking a qualified Nurse Practitioner specializing in Palliative Care to serve patients throughout the Hampton/Allendale area. The provider will deliver exceptional patient care across multiple settings, including homes, facilities, and telehealth, and collaborate with a multidisciplinary team to ensure comprehensive patient management.
Employment & CompensationThis is a full‑time salary‑based position with monthly bonus opportunities. Shift options: 8‑hour (Mon‑Fri 8:00–5:00) or 10‑hour (Mon‑Thu 7:00–6:00).

- Evaluate high‑risk patients with complex medical needs and develop individualized care plans.
- Coordinate care activities with the interdisciplinary team to ensure continuity and optimal patient outcomes.
- Communicate regularly with case managers, clinical coordinators, social workers, specialists, pharmacists, health coaches, and other team members.
- Participate in interdisciplinary team meetings to discuss patient progress, treatment plans, and care coordination.
- Foster effective communication and collaboration to facilitate seamless patient management.
- Meet staff efficiency expectations while providing high‑quality care in homes, facilities, telehealth, clinics, and specialties.
- Document care management activities accurately to support patient care plans and regulatory compliance.
- Provide education and resources related to patients’ conditions, treatment plans, self‑management, and preventive care.
- Offer support and guidance to patients and families to enhance understanding of healthcare options.
- Adhere to all relevant healthcare regulations, including HIPAA and Medicare guidelines.
- Monitor quality measures and gaps in care, identify opportunities for improvement, and implement corrective actions.
- Valid Nurse Practitioner, Physician Assistant, or Physician license in good standing in the state of employment.
- Experience in patient care, preferably in home health, clinic, facility, telehealth, or specialty setting.
- Strong communication, collaboration, and interpersonal skills for team environments.
- Ability to evaluate high‑risk patients, develop care plans, and coordinate care activities.
- Commitment to compassionate, patient‑centered care and a culture of excellence.
- Strong organizational and time‑management skills, able to multitask and prioritize.
- Strong written and verbal communication skills.
- Basic computer proficiency.
- Leadership skills and ability to foster a cooperative work environment.

- Flexibility to work varied hours, travel between offices and facilities, and hold a valid driver’s license with insured vehicle.
