Crescent Hospice is hiring a Full Time Hospice RN Case Manager (RNCM) to join our nursing team delivering exceptional end-of-life care in the community. Expect a 8 am–4:30 pm schedule, Monday–Friday, with a $5,000 sign-on bonus and generous benefits.
You will coordinate services, guide patients and families, and provide hands-on hospice nursing care within our interdisciplinary team. Prior RN experience and EMR proficiency are valued as you help ensure The Best Care Possible.
