Primary Care Nurse Practitioner

We are seeking Advanced Practice Providers for our Columbia, SC location. This position offers a unique opportunity to work in a collaborative environment, providing high-quality care with a focus on value-based outcomes.

• Monday-Friday, 8am-5pm schedule (no nights or weekends)
• Call schedule rotated amongst market, on average 1 week every 3 months (less frequent as more clinics open)

• Full-time permanent position with a 4-6 week orientation/training period

• Provide comprehensive primary care to geriatric patients
• Collaborate with a multidisciplinary team to ensure high-quality patient outcomes
• Utilize telemedicine and EMR systems effectively
• Participate in call rotations and contribute to team-based care initiatives

• Board Certification Required
• SC State License Required
• Training as a Family NP or Adult NP required
• Previous primary care or skilled nursing facility/nursing home experience preferred, but not required
