qualifications: nursing assistant certified by the state of south carolina, active and in good standing.

responsibilities:

shift starts at 2:30 pm to 11:00 pm.

check group assignment at main desk.

go directly to your assigned hall, make rounds with the outgoing shift to learn about your residents' status, make sure residents are dry prior to accepting the assignment, report to your nurse if any problem arises.

check your shower list.

make sure you have residents bathed and dressed properly before taking to the dining room.

feed on the hall then go to designated dining room or hall.

all residents should have bath or shower as scheduled. If resident refused shower, let the nurse know.

residents should be properly groomed, clothes should be matched and according to weather, nails are clean, shave as needed.

report to your nurse if resident refused to eat.

give 3 pm snack and record.

give ice if there is no resident assistant.

report any unusual incidents such as falls, skin tears, bruises, different behaviors and anything that is not normal for resident.

make sure to keep your rooms cleaned.

turn and reposition residents as scheduled.

pcr books should be completed before the end of the shift; shower and snack lists should be signed.

keep all closets cleaned.

clean wheelchairs, gerichairs as needed.

assist residents to and from the activity and the dining room.

keep residents' personal belongings labelled and bagged.

check vital signs weekly and prn.

complete meal records.

take dirty barrel out and bring an empty barrel for next shift.

follow chain of command.

perform mouth care before putting resident to bed. Remove and soak dentures.

make sure all residents are in bed clothes prior to putting to bed.

you are responsible for knowing proper restraint and treatments for your resident.
