Gentiva Hospice is seeking a dedicated Hospice RN / RN Case Manager to lead patients and families through the hospice journey in a home-based setting. You will deliver skilled nursing care, document care plans, and collaborate with an interdisciplinary team to update plans of care and ensure high-quality, compassionate support.
Ideal candidates hold an active RN license, CPR certification, and at least 1 year of RN experience (hospice or palliative care preferred).
