Supports Coordinator (Early Intervention Services)
Position SummaryThe Supports Coordinator (Service Coordinator) provides ongoing service coordination for children (birth to age
6) with developmental delays or disabilities and their families. This role ensures families can access needed services, understand their rights, and receive coordinated, high-quality early intervention supports that promote child development and family capacity.
Serve as primary point of contact for families
Assist families in accessing needed services
Coordinate and monitor service delivery
Make referrals and schedule appointments
Conduct follow-up to ensure services are provided
Assessment & PlanningCoordinate evaluations and assessments
Facilitate IFSP/FSP development and reviews
Participate in multidisciplinary team meetings
Help families identify goals and outcomes
Family Support & AdvocacyEducate families on rights and safeguards
Promote caregiver competence and empowerment
Support services in natural environments
Service MonitoringMonitor services to ensure alignment with IFSP/FSP
Coordinate across service systems
Support transition planning
Documentation & ComplianceMaintain accurate records
Complete service notes and documentation
Ensure compliance with Medicaid and IDEA Part C
CollaborationWork with families, providers, and agencies
Participate in team planning and communication
Qualifications- Bachelor's degree in one of the following fields or related area:
- Social Work, Psychology, Sociology
- Early Childhood Education or Special Education
- Child Development, Family Studies, or Health-related field
- Communication Disorders or Rehabilitative Counseling
- Minimum 1 year of experience working with infants, toddlers, or children with disabilities (Alternative combinations of education and experience may apply).
Preferred:
- Experience with Early Intervention, IDEA Part C, or pediatric services
- Knowledge of IFSP/FSP development and child developmental milestones
- Experience working with families and community-based services
