Job Duties- The APS Case Manager performs professional case management services for vulnerable adults in accordance with DSS policy and the APS ComPASS Model. Assesses risk, implements protective interventions, and coordinates services to ensure client safety while respecting the individual’s right to self-determination. Manages an assigned caseload, collaborates with community partners and legal systems, and supports emergency response efforts to maintain continuity of care.
Provides assistance to other counties as needed. - Manages an assigned caseload of vulnerable adults, including client contact, home visits, service planning, and coordination of legal actions as appropriate. Conducts timely assessments to determine risk factors such as abuse, neglect (including self-neglect), and exploitation, and implements protective interventions while honoring client choices. Maintains accurate, objective, and timely documentation in electronic case systems.
- Develops and coordinates service plans in partnership with clients to promote safety and stability, including arranging temporary and long-term placements and community-based supports.
- Collaborates with law enforcement and the court system to support emergency protective actions and assists clients in securing appropriate housing, placement, or protective arrangements.
- Coordinates communication among clients, families, providers, and internal staff to support effective service delivery and maintain professional working relationships.
- Assesses, in coordination with medical and legal professionals, clients’ capacity to access long-term supports, including financial resources, guardianship/conservator-ship, emergency services, and court involvement.
- Supports emergency response operations, including Red Cross sheltering and disaster-related services; assists in developing and monitoring client emergency plans.
- Performs other related duties as assigned.
- A Bachelor's Degree in Social Work, Behavioral Science, or Social Science.
- A Bachelor's Degree in any other field and one (1) year experience in a related field.
- Must possess a valid driver's license and have access to and be able to operate a motor vehicle. Individuals must comply with necessary field work assignments.
